What causes erectile dysfunction?
In the majority of males, erectile dysfunction is caused by vascular disease. Risk factors for vascular disease include diabetes, hypertension, hypercholesterolaemia, and smoking.It makes sense that when our patients do have these risk factors, they are referred across for a cardiac assessment prior to delving into options for treating erectile dysfunction.
Aside from assessing for vascular disease, we look at the other causes of erectile dysfunction, including hormone deficiencies, prostate cancers, psychological factors.
Treatment will involve
Lifestyle Modifications: Addressing lifestyle factors such as smoking cessation, weight management, and regular exercise can positively impact ED. Adopting a heart-healthy lifestyle can improve overall vascular health.
Hormone Therapy:In cases where hormonal imbalances are identified, hormone replacement therapy may be considered to restore testosterone levels.
Oral Medications: Phosphodiesterase type 5 (PDE5) inhibitors, such as sildenafil (Viagra), tadalafil (Cialis) may be prescribed. They enhance the effects of nitric oxide, promoting increased blood flow to the penis.
Injections: For those who do not respond to oral medications, injectable medications may be used to stimulate erections. These are ordered on a per patient basis.
Vacuum Erection Devices: Mechanical devices, such as vacuum erection devices, create a vacuum around the penis, drawing blood into the area to induce an erection. A constriction band is then placed at the base to maintain the erection. These are helpful in certain circumstances and may not suit all men.
Penile Implants/Penile Prosthesis surgery:Surgical options include the insertion of penile implants, which are inflatable or semi-rigid devices placed within the penis to enable erections. This is a more invasive but effective solution. Dr Bugwandin has an interest in penile prosthesis surgery and is able to offer these surgical options to patients who are interested in proceeding.

Psychotherapy and Counseling: Addressing psychological factors through counseling or psychotherapy can be beneficial, especially when stress, anxiety, or relationship issues contribute to ED.
