The prostate is a gland that sits at the exit of the bladder. In a young man it is 15-25 cc in volume. Imagine it as a ring donut, with urine passing through the donut hole in the centre. As one grows older, the donut often enlarges, and the hole at the centre closes off, obstructing the flow of urine.
Who needs to screen for prostate cancer ?
If one has a family history of prostate cancer, cancer screening should commence from age 40 and should be carried out annually.
Other males should be screened at age 40 and 45, and then annually from age 50
How do we screen ?
Screening for prostate cancer can be done at your family practitioner or urologist. A blood test(PSA) and physical exam are done, and in some cases, an MRI scan may be indicated.

How do I know that I have a kidney stone?
The commonest symptom of kidney or ureteral stones is flank pain, which is accompanied by microscopic blood in the urine
Bladder Prolapse may present with varying symptoms and signs, however very often, women notice a bulge of tissue in the vagina, with a range of urinary symptoms. Alternatively, a gynaecologist may make you aware of a bladder prolapse after a routine gynaecology assessment
Risk Factors for bladder prolapse include :
Advancing Age, Obesity, Constipation, Chronic cough, Previous vaginal deliveries/trauma, Gynaecological surgery
We broadly divide urinary incontinence into stress incontinence, urge incontinence, and mixed incontinence.
Stress incontinence refers to incontinence experienced when the intra abdominal pressure rises- such as with coughing, laughing, exercising, heavy lifting etc.
Treatment involves:
Pelvic floor exercises; Bulking agents; Vaginal tape surgery(Transobturator or Retropubic tapes)
What causes erectile dysfunction?
In the majority of males, erectile dysfunction is caused by vascular disease. Risk factors for vascular disease include diabetes, hypertension, hypercholesterolaemia, and smoking.It makes sense that when our patients do have these risk factors, they are referred across for a cardiac assessment prior to delving into options for treating erectile dysfunction.
Peyronie's disease is a condition that can affect the shape and function of the penis.
Dr Bugwandin uses a combination of medical therapies(Pentoxyfilline) and physical therapies(Traction therapy, ESWL Shockwave Therapy) to help reverse your disease. If mild, then these therapies are enough to restore you to normality. In severe cases, plication surgery or penile prosthesis surgery may be offered to restore form and function.
Strictures are essentially scars that may occur in the urethra or ureter.
Male urethral strictures are extremely common. Traditionally, they are treated with either incision(Urethrotomy), or open surgery(urethroplasty). Aside from these traditional methods, Dr Bugwandin offers cutting edge Urethral stenting & Optilume Therapy, which reduce the recurrence of urethral strictures.
Neonatal(Baby) circumcisions are generally performed for religious, cultural or medical reasons. Dr Bugwandin performs these procedures in his room or in the maternity ward of the hospital. It is best performed at a younger age(ie. Ideally within three days of birth), however we are able to perform these procedures for older babies as well if required.

Blood in the urine can be a manifestation of a variety of diseases. In the younger patient it is often seen during infection, or when a urinary stone is present, however in the older patient, over 40 years of age, we would need to exclude a urinary tract cancer. Bladder cancer and Kidney cancers may have Haematuria as their only symptom. In most cases, a CT Scan and a Cystoscopy are mandatory to assess the patient with haematuria.

Dr Santosh Bugwandin qualified as a urologist in 2011, and has an interest in BPH/minimally invasive prostate surgery (HoLEP/Rezum), female urology, laparoscopic surgery and laser lithotripsy for kidney stones.
Since undergoing HoLEP training internationally in 2014, he has performed hundreds of HoLEP procedures with excellent outcomes. He is a HoLEP proctor/trainer who teaches other urologists this procedure. We manage complex referrals from across the region, utilising the world’s most advanced laser technology, a Lumenis P120 Moses2 Holmium Laser.
Aside from HoLEP, Dr Bugwandin regularly performs Rezum Water Vapour Thermal Therapy for BPH. Dr Bugwandin amalgamated practices with Dr Douw Steyn (a urologist with decades of experience in female urology) in 2024. He benefited from two years of direct surgical mentorship, and further advanced his skills in advanced female pelvic-floor reconstruction in Germany, in 2024. Dr Bugwandin offers urodynamic testing in room, and performs a range of pelvic floor surgery for female pelvic organ prolapse, and urinary incontinence.
Dr Bugwandin also holds a Certificate in Minimally Invasive/Laparoscopic Surgery from the University of Strasbourg, reinforcing his expertise in precise, modern surgical techniques.
